# 345: Review
if the nurses notes revealed that no mention was made
of the blood sugar values for the patient. The patient
began hallucinating. The doctor was notified. Orders
were received and carried out. The patient went into
cardiac arrest. His blood sugar value was critically
low. Discovered the facts leading to the arrest- critically
low blood sugars that were never addressed.
Case
# 764: Patient was transferred to a nursing
home facility. Orders were written for several medications.
No medications were administered for over 24 hours
after admission. Prior to discharge from hospital,
blood levels for certain medications were less than
therapeutic. Patient subsequently suffered a seizure
and ended up back in the hospital with brain damage.
Helped identify the lapse in medication administration
and low drug levels leading to seizure activity.
Case
# 1972:
Patient was involved In a motor vehicle accident-
hit from the rear. Car sustained minimal damage. Patient
suffered several herniated discs to the low back.
Patient had no prior history of low back pain. Researched
issue and identified studies revealing that low impact
injuries (whiplash) can indeed cause injuries to this
extent.
